The unfortunate incident for Zouma happened just before the hour mark after he rose high to head the ball in the centre of the pitch. However, as he went to plant his right leg on the Stamford Bridge turf it buckled beneath him in an unnatural angle - as he landed on his heel as opposed to the ball of his foot.
As pictures taken by Sportsmail photographer Andy Hooper show, Zouma's right leg bent unnaturally away from his body.
The Chelsea centre back's screams could be heard loud and clear by the television cameras in distressing scenes for Zouma.
After being attended by the Blues' medical staff he was carried off the pitch on a stretcher as the home fans showed their support with a rousing applause.
England international Gary Cahill took his place in the heart of the Chelsea defence.
United left back Luke Shaw, who hasn't played for the club since breaking his leg at PSV Eindhoven in September, took to Twitter to express his support for Zouma.
'Best wishes to @KurtZouma get well soon!' he tweeted.
